Structured Family Care (SFC) is an Indiana Medicaid program that allows eligible family members or loved ones to receive financial support while caring for someone at home.
Under Structured Family Care, the caregiver and the person receiving care must reside at the same address.
✅ Provide Care
As the caregiver, you provide daily support for Activities of Daily Living (ADLs) like bathing,
meal preparation, housekeeping, transportation and more.
✅ Waiver Eligible
This service is designed for individuals who qualify for Indiana’s Health & Wellness, PathWays, or Traumatic Brain Injury (TBI) waivers.
Your care team includes dedicated Care Managers and Care Coaches who provide ongoing guidance, answer questions, and support you through regular in-home visits.
