As a SFC caregiver with Indiana Family Caregivers
Under Structured Family Care, the caregiver and the person receiving care must reside at the same address.
As the caregiver, you provide daily support for Activities of Daily Living (ADLs) like bathing, meal preparation, housekeeping, transportation and more.
This service is designed for individuals who qualify for Indiana’s Health & Wellness, PathWays, or Traumatic Brain Injury (TBI) waivers.
Your care team includes dedicated Care Managers and Care Coaches who provide ongoing guidance, answer questions, and support you through regular in-home visits.

Our team helps guide paperwork, care planning, and documentation.
After services begin, we stay connected with caregiver coaching, check-ins, and care coordination.
